Summary:

The RN Program Manager will be focused on successfully driving the launch, adoption, and subsequent refinement of Somatus technology products in our proprietary suite of software tools, RenalIQ. This position will be required to curate and disseminate specialty-related deliverables, including workflows, user guides, marketing collateral, and assist with roadmaps and requirement documents.

Essential Duties and Responsibilities:

+ Acts as an advisor to the executive leadership

+ Serve as the Subject Matter Expert providing input to identify best practices to meet organizational goals and for efficient and effective utilization of RNCM staff and resources

+ Provide guidance and design training programs for new and existing RNCM teams

+ Collaborate with operational directors and managers to design, create, and systematize processes for measuring performance outcomes and key performance indicators

+ Identify staff training needs and ensure the development and implementation of programs that address needs in collaboration with operational leaders

+ Provides guidance to create, review, and maintain discipline-specific standard operating procedures (SOP)

+ Know and comply with all state and federal regulations relating to the RN Care Manager scope of practice

+ Maintain current knowledge of policies and regulations in the RN Care Manager field, especially related to kidney care

+ Develop program material and outreach materials, and update regularly to ensure the accuracy of the information, and evaluate the effectiveness on an ongoing basis

Required Education and Experience:

+ Licensed Registered Nurse

+ Bachelor's Degree in a healthcare-related field

+ Three (3) to five (5) years of program management experience or operational experience in population health, managed care, case management, or healthcare consulting

+ Three (3) to five (5) years of hands-on care management experience in a patient setting

+ Experience working with an outcome-based program using data to drive performance improvement strategies for both program services and personnel.

Preferred Experience:

+ Six (6) months to one (1) year experience as Somatus Ops Manager or RNCM.

+ Renal/Dialysis experience

+ Experience working with patient and provider engagement platforms

+ Experience with data analysis, preferably with usage logs from digital health applications

+ Experience implementing digital health products

+ Advanced knowledge of case management and care coordination of services specific to the Somatus clinical model

Knowledge, Skills, and Abilities:

+ Effective presentation skills with the ability to clearly present ideas, information, and viewpoints, both verbally and in writing.

+ Demonstrated ability to adapt to changing needs by acquiring new skills and Knowledge

+ Demonstrated knowledge in Kidney care and ability to understand the RN Care Manager's role in managing patients with kidney disease

+ Acute attention to detail and the ability to create and drive project plans

+ Ability to be innovative and navigate ambiguity

+ Self-starter, highly organized and detail-oriented with the ability to take initiative and execute multiple priorities

Physical Requirements

+ This job operates in a professional setting. While performing the duties of this job, the employee is regularly required to sit or stand for extended periods of time. Normal manual dexterity is required.

+ Normal speaking and hearing abilities to interact with others in an office environment, over telephone or other video conferencing platform.

+ The employee is occasionally required to stand; walk; and reach with hands and arms and continuously repeat the same hand, arm finger motion many times as in typing.
