Harbor Care, formerly known as Harbor Homes and the Partnership for Successful Living affiliates, is an innovative health and human services non-profit organization that provides housing, healthcare, home health, behavioral health, addiction treatment, and more in collaboration with many community partners. Harbor Care is the new shared name of Harbor Homes, Keystone Hall, Healthy at Home, the Harbor Care Health and Wellness Center, and the Southern NH HIV/AIDS Task Force. As the Greater Nashua region’s only independent non-profit home care agency, more than any other similar entity, Healthy at Home provides home care services to the most vulnerable in our community, particularly to those whose insurance runs far short of covering all costs.


Since 1997, the agency has helped thousands of individuals stay in their homes and avoid the expense and risks of assisted living facilities, nursing homes, hospitals or other institutions—benefiting clients, families, insurers, and the entire community. No client is asked to pay above what insurance provides, and Harbor Care, a nonprofit agency, annually provides $300,000 in subsidies. Moreover, through local affiliations and partnerships, we ensure clients receive all the supports they need to succeed.

Job Summary:

Assist client with non-medical daily living activities at the client’s home. Duties performed at a place of residence may include but not limited to keeping house (making beds, doing laundry, washing dishes) and preparing meals and providing personal care. He/she functions under the direction, instruction and supervision of the RN assigned to the case, Director of Client Services or Administrator. Healthy at Home is unique in providing staff:

Educational and Professional:

  • High School Diploma or GED
  • Successful completion of PCSP training program, which meets with State and agency regulations.
  • Show evidence of fulfilling the State health requirements.
  • Current CPR certification.
  • Valid drivers’ license and auto insurance
  • Must be 18 years or older.
  • Successfully complete 12 in-services by December 1 of current year
  • 6 Hours initial of Dementia Training and 4 hours yearly continued

Knowledge and Abilities:

  • Know home care rules and regulations
  • Be aware of and adhere to all HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency
  • Good oral, written and reading communication skills. Have the ability to understand verbal and written instructions. Read and write documentation.
  • Resourcefulness, flexibility and integrity. Ability to work independently. Ability to function in a team-oriented environment
  • Ability to make meaningful observations and write brief and accurate reports when necessary

Responsibilities:

  • Assist the client with personal hygiene (bathing, shampoo, shaving and oral hygiene)
  • Plans and prepares nutritious meals or modified diets as prescribed by a physician.
  • Assists with activities of daily living which include dressing, ambulation, toileting, and safe transfer techniques as directed by the RN or PT.
  • Prompts client to take self-administered medications as per the care plan.
  • Works with other community agencies involved with the client as directed by the RN or PT.
  • Assists with treatments/therapies as ordered by physician and approved and supervised by RN or PT.
  • Carries out assignments as instructed by LNA care plan and notifies RN or PT when unable to do so. If RN or PT is unavailable, notify the Director of Client Services.
  • Reports changes in clients’ condition or home situation immediately to the office.
  • Use of IPAD for electronic record of visit, including activity, time and mileage
  • Performs routine housekeeping duties as related to a safe and comfortable environment for the client as directed by the RN or PT.
  • Accompany the client to necessary appointments, only when authorized by the office.
  • Respect the cultural and religious practices of the client and their families.
  • Maintain a neat, clean and orderly appearance *(Dress Code Policy)
  • Appropriate footwear for all seasons *(Dress Code Policy). Understands and maintains universal precautions.
  • Performs other duties as required by Director Client Services, Administrator, or designee.
  • Conduct oneself in a manner that promotes safety

Hoyer Lift Glucose monitors or meters - Glucometers

Gait Belt Therapeutic ice packs or pillows – Ice caps; Ice collars

Sharps disposal Walker

Wheelchair Cane

Tub seat Transfer/slide board


Physical Demands:

  • Lifting, carrying, pushing or pulling up to 50#.
  • Patient transfers with no assistance for direct patient care.
  • Standing, walking, stair climbing, stooping, kneeling and crouching while administering patient care in the home.
  • Reaching and handling
  • Adequate eyesight for reading, writing, driving and performing tasks.

Aptitudes:

Have the ability to understand verbal and written instructions. Read and write documentation in patient record. Work with the hands and fingers to manipulate large and small objects.
