Job summary
The Resident Care Manager is responsible for ensuring that all residents receive the proper care, encouragement and support, and to live their best life possible and optimize their continued wellbeing.
Responsibilities and major duties:
- Overall responsibility for specific resident population that includes comprehensive understanding of their needs, communication with Charge Nurses, maintaining accurate and timely Care Plans, effective communication with residents and family as well as all necessary support personnel and 3rd party providers –
- Oversight of all 3rd party clinical support services such as Medical Director, PT, OT, Hospice providers, etc.
- Provides oversight of the community medication management to promote resident safety in medication use process.
- Backup to Director of Nursing for weekly timecard reviews & adjustments and staffing agency timecard reconciliation.
Assessments:
- Review and collaborate on all prospective resident’s clinical assessments to confirm the appropriateness of fit within the WSL community.
- Work collaboratively with Admissions and Marketing department to efficiently coordinate admissions, discharges and transfers by coordinating the clinical requirements for these processes.
Clinical:
- Champion resident rights & include them in the development & implementation of their Care and Service Plans.
- Ensure that residents and/or responsible party (as appropriate) participate in the completion of the assessment and development of the resident care plan.
- Responsible for creating Care Plan and coordinating the execution of Person-Centered resident care services in collaboration with the Health Resources team, support providers, residents and families.
- Ensure that Care Plans, 30 & 90-Day Progress Notes, Assessments and any other documentation are complete, accurate and updated as needed. May delegate to Charge Nurses who have the required training.
- Monitor resident acuity and evolving levels of care needs to ensure appropriate care plans are in place and proper fees are being charged.
- Schedule, host and meet with residents and families routinely and during the Care Plan meetings, executing decided upon action plans.
- Oversees resident care to ensure that all residents have the opportunity to live their best life possible while preventing unnecessary decline and/or acute episodes.
- Work collaboratively with the Charge Nurses to ensure that the plan of care is delivered accurately and consistently in accordance with the resident’s wishes and abilities.
- Keep residents and families aware of significant changes in resident condition.
- Be the lead contact for coordination of care for Primary Care Provider consults, Behavioral Health, PT, OT, Hospice and other healthcare professionals utilized for Coveside Commons Memory Care and All Points and Enhanced Enrichment residents.
- Establish and maintain positive, professional relationships of trust and respect with all members of the community, residents, families, and staff.
- Address high level or unresolved resident issues when escalated from the Charge Nurse.
- Prepare clinical notes and prioritize resident visits with Medical Director / Physician as needed. Ensure follow-up notes and orders are updated in resident charts in a timely manner.
- Maintain working knowledge of RAI process to ensure successful resident outcomes to align with our age in place community.
- Work collaboratively with the Charge Nurses to resolve resident and/or family concerns. Escalate high level or unresolved resident issues to the Director of Nursing or VP Resident Services.
Operations:
- Embrace the culture of continuous improvement / optimization and promote decisions to transition to new systems and processes through the establishment of state-of-the-art technology for progressive communication, paperless records, etc.
- OnShift, PCC, Lifeloop and other software programs and technology experience preferred.
- Maintain and update policies and procedures as necessary in order to meet or exceed current standards of care.
- Ensure that resident medical records are complete, up to date and compliant, including but not limited to the following:
- Medical records from ancillary providers
- Obtaining MD signatures for telephone orders as needed
- Scan, organize and file documents daily for individual resident files.
- Creating new resident charts & new resident documentation in PCC.
- Audit / update resident EMS packet as needed
- Maintain effective communications with Director of Nursing.
- Maintain effective lines of communication with staff for clear and ongoing transparency.
- Willing to collaborate and work well with the entire WSL team as well as Residents.
Personnel:
- Participates in the interview process with Human Resources as needed.
- Ensure that staff understand their responsibilities, has the necessary training to execute it, and is held accountable for their performance.
- SCHEDULING – Support Scheduler and Director of Nursing in creating and managing Charge Nurse, MNA, RA/LNA scheduling and floor assignments to ensure optimized staffing. Provide support as needed when a callout occurs or when staffing levels fall below WSL expectations or 805 requirements.
- Will be required to work 1 Charge Nurse shift per week to maintain their nursing skills, be available as needed for emergency back-up, and more importantly, to have a meaningful connection with residents and all clinical staff. This is a major cultural focus as all members of the WSL team maintains the willingness to do whatever it takes to successfully support WSL operations.
- Educate Resident Care Team members on proper time off requirements & expectations.
- Adheres to the company’s attendance policy and understand that their attendance is a vital component to their job. This position is a critical on-site function, so remote work would be managed by exception.
- Must establish a productive, positive and professional work environment that is imbued with excellence as evidenced by ongoing staff training, growth, and development.
- Assist with the orientation and onboarding process of new employees, as needed.
- React urgently to personnel issues and maintain open communications with HR to keep them abreast of situations that may require escalation or increased support/engagement.
- Provide timely follow-up on all employee/resident issues and/or concerns. (Personnel)
- Complete employee performance evaluations on time and with valuable feedback on employee contributions and areas of improvement.
- Other duties as assigned.
EDUCATION AND EXPERIENCE
Bachelor’s or Associate degree in nursing preferred. Three or more years of demonstrated leadership in a senior service setting preferred.
LICENSURE, REGISTRATION AND/OR CERTIFICATION
Current State license as Registered Nurse (RN) or Licensed Practical Nurse (LPN).
