About this role:
The Social Worker at Hampstead is a vital member of the clinical team, providing therapeutic and case management services to patients and their families. This position collaborates closely with aftercare providers and other stakeholders to ensure a seamless continuum of care. The Social Worker at Hampstead conducts individual, family, and group therapy as needed, actively participates in treatment planning, and develops psychosocial formulations while coordinating with internal and external stakeholders to ensure a smooth transition plan for patients.
What you bring to the table:
Education
  • Master’s or doctoral degree in Social Work, Mental Health Counseling, Psychology or related field from an accredited graduate program.
Experience
  • Related field work experience and/or previous experience with patients/families in a psychiatric setting.
Licenses/Certifications
  • Licensed or license eligible in New Hampshire.
  • License eligible clinicians are required to pursue licensure.
  • CPR Certification (within 3 months of hire)
  • CPI Certification (within 3 months of hire)
Compliance
  • Corporate Compliance (within 2 months of hire)
  • Aligned with NH’s System of Care, supporting services that are: family-driven, youth-guided, community-oriented, trauma-informed, and culturally and linguistically competent.
What you will do:
  • Provide direct and indirect therapeutic services, including group, individual, and family therapy, case management, psychosocial assessments, aftercare planning, and facilitating systems meetings.
  • Coordinate discharge planning and continuing care plan in accordance with treatment team recommendations and provide clinical consultation to other team members.
  • Develop and maintain professional relationships with community healthcare providers, providing consultation to collateral outpatient providers, residential programs, and schools as needed.
  • Perform level-of-care assessments and pre-certification reviews with insurance companies, and complete documentation in accordance with Joint Commission, State Licensing, and Medicare standards.
  • Promote a culture of patient safety, report medical errors and significant events, and may provide direct clinical supervision to less experienced staff. This position may require flexible hours, including nights and/or weekends, to accommodate patient and family needs.
